This fully-automated, quality-driven utilization management (UM) solution empowers health plans to seamlessly transition specialist groups toward value based care by directly rewarding optimal performance.

- MedMarkers™: process-of-care quality measures embedded within clinical guidelines that identify key services driving variations in specialist practices.
- Clinical MedMarker Protocol Ranges™: achievable, evidence-based boundaries for clinical practice. Utilization below the lower threshold indicates potential under-utilization of care, while utilization exceeding the upper threshold signals over-utilization.
By replacing administrative friction with transparent, clinically-driven tiers, QualiTier™ aligns health plan and specialist goals to lower healthcare costs and improve patient outcomes. The process works as follows.

A clinical row consists of a medical condition episode of care with an assigned severity-of-illness (SOI) level, MedMarker™, associated Clinical MedMarker Protocol Range™, and a target threshold – which generally is the upper-end of the Clinical MedMarker Protocol Range™.

- Tier 1: represents the highest-quality, more efficient specialty groups that meet or exceed the required Clinical Score pass threshold.
- Tier 2: assigned to groups meeting intermediate quality and efficiency standards.
- Tier 3: assigned to groups exhibiting lower clinical adherence and higher-utilization treatment patterns.
For example, Ear, Nose, and Throat (ENT) specialists assigned to Tier 3 routinely perform higher rates of nasal endoscopies (CPT code range: 31233-31298). Following targeted education regarding these practice variations, health plans can now adjust payment structures for high-utilization procedures to incentivize more optimal, value-based performance.
By introducing clinical transparency to network tiering, QualiTier™ bridges the gap between insurers and providers to foster a higher-value healthcare system. By adjusting payments to Tier-3 specialists, a health plan will receive a minimum 10:1 return for implementing CCGroup QualiTier™.
