Cosm Medical Achieves Reimbursement Pathway Milestone: US CPT® Codes Recognize the Work Behind Personalized Pessaries

New US coding recognizes the clinical work of designing gynecological devices from a woman’s own anatomy, opening a path to personalized care for the many women who struggle with standard, off-the-shelf devices, like pessaries.

TORONTO–(BUSINESS WIRE)–#3DPrintingInHealthcare–Starting January 1, 2027, US clinicians will, for the first time, be able to report the work of designing a pessary personalized to a patient’s anatomy. The American Medical Association (AMA) has revised CPT® Category III codes 0559T and 0560T following a code change application co-led by the American College of Radiology (ACR) and Cosm Medical Corp. (https://www.cosm.care), a medical technology company specializing in personalized gynecological devices.

Nearly half of all women will experience a pelvic floor disorder such as pelvic organ prolapse or stress urinary incontinence in their lifetime,1 and about one in five will have surgery for one of these conditions.2 For women who want to avoid or delay surgery, a pessary is a first-line, non-surgical treatment. Yet standard, off-the-shelf pessaries cannot be successfully fitted in approximately 30% of patients,3 and 33–38% stop using their pessary within a year,4,5 often because the device does not fit their anatomy well. Surveys of urogynecologists highlight challenges in achieving an optimal pessary fit, reinforcing the need for more personalized fitting strategies and designs6, such as those offered by Cosm Medical.7

The CPT® codes formally recognize, within the US coding system, the clinical work of designing a device from a patient’s own anatomy rather than selecting a standard shape.

For reference, the revised code descriptors read:

0559T – Patient-specific anatomic model or structure 3D-printed from image or anatomical data set(s); first individually prepared and processed component.

+0560T – Patient-specific anatomic model or structure 3D-printed from image or anatomical data set(s); each additional individually prepared and processed component (List separately in addition to code for primary procedure)

The 2027 revision broadened the codes to describe a 3D-printed patient-specific anatomic model or structure — language that encompasses a personalized Gynethotics™ pessary, which is designed and fabricated to the anatomy of the individual patient.

“Orthopaedics and dentistry made patient-specific devices routine decades ago. Pelvic health is next. Too many women are still handed a standard pessary and asked to make it work, and many stop treatment when it doesn’t fit,” said Derek Sham, Founder and CEO of Cosm Medical. “Working alongside the American College of Radiology to have personalized devices recognized in the CPT® code set is an early but real step toward care designed for each woman’s anatomy.”

Cosm Medical partnered with PRIA Healthcare, a medical device reimbursement, market access, and regulatory consultancy, to develop and support the CPT® code change application.

To help clinicians and their billing teams understand how the new codes apply, and how they relate to existing pessary coding, Cosm Medical has published a coding reference on its website: cosm.care/reimbursement.
