NASHVILLE, Tenn.–(BUSINESS WIRE)–Together Health, a modern primary care company delivering in-person care in the home or workplace, with virtual visit options, today announced its continued expansion across major metropolitan areas in Tennessee and into Seattle, Washington. The company’s care model removes traditional barriers to access by bringing healthcare directly to patients rather than requiring patients to travel to clinics.
Founded by Huy Tran, Together Health was created to address persistent challenges in the U.S. healthcare system, including rising employer healthcare costs, clinician burnout, and limited access to trusted, affordable primary care.
Prior to founding Together Health, Huy spent years as an Human Resources Executive, where he consistently observed healthcare premiums increase year over year with limited improvement in employee experience or health outcomes. At the same time, clinicians faced growing administrative burden, burnout, and turnover, further constraining access to care. According to a recent Mercer study, employer health benefit costs are projected to rise 6.7% in 2026, underscoring the need for more sustainable care models.
“Rising costs, limited access, and clinician burnout are deeply connected challenges,” said Huy Tran, Founder and CEO of Together Health. “Patients, clinicians, and employers experience the consequences of these issues every day.”
Huy’s motivation to build Together Health was also shaped by personal experience. While helping his mother-in-law navigate the healthcare system, he encountered long wait times, fragmented care, and barriers to access that made receiving timely, trusted care difficult. That experience reinforced the need for a simpler, more transparent primary care model.
“Together Health is addressing some of the most pressing structural issues in healthcare by aligning incentives across patients, clinicians, and employers,” said Keith Payet, CEO of UnitedHealthcare Community & State of Washington and Board Member of Together Health. “Its focus on access, affordability, and clinician sustainability represents the type of innovation our healthcare system needs.”
Together Health delivers primary care through a membership-based model designed to be affordable and transparent for individuals and employers. Care is delivered in person—at home or in the workplace—with virtual visits available, allowing patients to receive care on their terms while supporting clinicians in practicing relationship-based, whole-person medicine.
Together Health now serves major metropolitan areas across Tennessee, including Nashville, Memphis, Knoxville, and Chattanooga, and has expanded operations to Seattle, Washington.
